Dear Parent/Guardian:

 

Permission for medication administration automatically expires at the end of every school year for all medications.  

 

Please remember to pick up any remaining medication before the last day of school. This must be done at the end of every school year. Medication which has not been picked up will be discarded at the completion of the last school day.

 

Medication orders must be renewed prior to the start of every school year. To insure a smooth transition into the next school year,  please download the Medication Authorization Form from the WPS website under the Nursing Department tab. The form must be signed by both the parent and the student’s health care provider. Every medication, both prescription and over-the-counter, require completed forms before being dropped off and/or administered at school. Other forms are also available on the website including an Allergy Emergency Care Plan and an Asthma Management Plan.

 

At the start of the new school year, please bring the medication in the original container that includes a pharmacy label (or manufacturer’s container for over-the counter-medication) and the proper documentation with required signatures to the nurse’s office.
