Dear Parents and Guardians,
This letter is to remind you that there is a new MA school immunization requirement. Students who are entering 11th grade must have one booster dose of MenACWY (meningococcal conjugate vaccine) that has been given on or after 16 years of age. Please note that the Men B vaccine does not satisfy this requirement.
If your student has had a recent physical exam and received the MenACWY immunization, please send a copy of the immunization record to our office. You may mail, email, or fax the form. This vaccine is REQUIRED for attendance at school in the fall. Please follow up with our office if you have any questions.
