Students are reminded to complete their course requests in Powerschool and complete the verification sheet. If you wish to change your teacher recommendation, please complete this form, which requires signatures from your parent/guardian, your current teacher, and Department Head in the subject you are wanting to change. Completed forms should be dropped off at your House Office. **Incomplete forms will not be processed.
