Name & Contact Information Changes

Please complete the Employee Information Change Form for any of the following changes:

  • Name (First, Middle, Last)
  • Home address
  • Phone number
  • Email address

Upon completion, please forward to askhr@wellesleyps.org. 

Please note that name changes will require submission of an updated social security card to Human Resources. Please contact askhr@wellesleyps.org to schedule a time to drop off a copy of this document.
