By signing the acknowledgment form linked below, you grant the Wellesley Public Schools permission to conduct a CORI check. To withdraw your consent, provide written notice to Wellesley Public Schools. Information obtained from this check will be treated with strict confidentiality and used solely for the purpose of safety and security.
