MEMO: Medication Drop Off

Welcome back Wellesley Public School Families —
In an effort to limit traffic in our school building, the nursing department will be handling medication drop off for the 2020-2021 school year with two scheduled drop off opportunities:
September 22nd 12pm-2pm and
September 24th 9am-11am.
At PAWS, the nurse will be stationed by the front bench. Please have an adult bring the items listed below in a ziploc bag labeled with the student’s name and class. Drive up to the nurse’s station and stay in your car with your mask on, the nurse will complete a quick review of the bag contents. Incomplete packages will not be accepted, they will be returned to you to be resubmitted when all items have been collected.
A COMPLETE Medication package includes:

1. Non-expired medication in its original, pharmacy-labeled container
(No more than a 30-day supply of daily medications will be accepted.)

2. Completed WPS Medication Authorization Form signed by Primary Care Provider (PCP) and Parent/Guardian.

3. Action Plan (as applicable) signed by Parent/Guardian

Seizure plan* (provided to you by your PCP)

Diabetes* (provided to you by your PCP)
