- Educator sets two goals by November 1

- 1 student learning goal
- Evaluator observations ≥ (greater than or equal to) 4 observations
- ≤ (less than or equal to) 2 days Educator & Evaluator have a conversation
- ≤ (less than or equal to) 2 days Educator completes post obs. form
- ≤ (less than or equal to) 2 days Evaluator makes comments on form
- Mid-Cycle Assessment due by June 1 of the first year
- Summative Assessment due by June 15 of the second year

- Form 2 – Goal Setting Form – Due November 1, each year
- Form 3 – Post Observation Form – Due within 2 days of Observation (5 total)

- Form 5 – Summative Evaluation – Due June 15 of the second year
