Dear Parents,

The summer is winding down and we are looking forward to seeing all of your children on August 29th. As you know, we have a new school nurse, Christine Babicz. Ms. Babicz has been reviewing student health records and is committed to continuing to provide excellent nursing services at Sprague. To that end, she asked me to share the following note and attached forms with parents. She would like to get to know student health needs and have the Health Office up and running smoothly on the first day of school. That means she is looking for any missing medical records or updated immunization records that may be needed for school.

Welcome Back!  As the new school nurse at Sprague,  I’m very excited to meet all of you and get a jump start on the school year.  

My office hours prior to the start of school will be:

August 20   8-3

August 21   8-3

August 22  1:30 – 3:30

August 27   2:30 -4

August 28  10:30 – 3

  • For families with specific health care needs or health related questions/concerns, please email me to set up a time to meet prior to the start of school   

  • Please note that a medication authorization form will need to be completed for any medication needed during the school day.  I have attached the WPS Medication Authorization Form and the WPS Allergy Action Plan for your convenience.

  • If able, please drop off any medication, and/or completed medication forms, allergy action plans, or asthma action plans prior to the start of school.
